Primary Medical Care
Every medical patient at Fenway Health works with a primary care team, which includes a primary care provider and a registered nurse who are actively involved in your care. You may select a primary care provider at the main Health Center (1340 Boylston Street) or at South End Associates of Fenway Health (142 Berkeley Street). Depending upon your preference, your primary care provider may be a physician, physician's assistant, or nurse practitioner. You may request a male or female provider or a Spanish-speaking provider.
Your provider serves as a team leader and coordinates all routine medical care: exams, lab tests, medical imaging, diagnosis, and treatment. In addition, your provider is concerned about emotional and lifestyle factors—such as stress, diet, substance abuse, or high-risk sexual behavior—that affect your health.
Specialty Care/Referrals
Your Fenway Health provider may refer you to medical specialists at the Health Center. Areas of specialty include infectious disease, gynecology, psychiatry, gerontology, dermatology, nutrition, and podiatry. If a required specialty is not available at Fenway, your provider will refer you to a specialist at Beth Israel Deaconess Medical Center or through the CareGroup network.
If you would like to request a referral from your PCP to a specialist, please gather the following information:
- Your name, phone number and date of birth
- The name of your Primary Care Provider
- The name of the specialist you are requesting an appointment with
- The specialist's hospital, phone number and fax number
- Your insurance carrier and insurance ID #
- Date of next appointment
- Reason for referral
Immunizations
Fenway Health offers immunizations for many communicable of diseases. You can easily schedule an appointment for immunizations against:
- Hepatitis A & B
- MMR - Measels, Mumps and Rubella
- Meningitis
- Tetanus
- Polio
- Chicken Pox
In most cases, you don't need to see a medical provider to receive immunizations for these diseases. These immunizations are generally covered by insurance, but please check with your insurance carrier to confirm coverage.

Seasonal Flu Shot Clinics Fenway Health offers season flu shot clincs prior to the start of each winter flu season. These clincs are held several times during the fall months at both the main health center at 1340 Boylston Street and our South End Associates practice at 142 Berkeley Street. Once you have gathered the information, you can request a referral in one of three ways: 1.) Call the referral request line at (617) 927-6130 and leave a detailed message. 2.) Send an email to the Referral Department. 3.) Send a fax to the Referral Department's attention at 617.247.3460.
Travel Immunizations Fenway Health offers most of the immunizations or vaccines that those planning international travel may want or be required to receive. In most cases, it's necessary to see a medical provider to receive these immunizations.
